Thank you for your interest in a grant from the Center for Community Health Leadership, sponsored by Misys Healthcare Systems, and congratulations on New Haven's selection for a grant. Numerous communities applied for a grant from the Center, and your community was selected from more than 60 applicants. 

In the following pages, you will be asked for basic information about your organization to enable us to assess the software donation required to bring your clinicians live on electronic health records. We will need information about the size and volume of your organization, as well as your current technical capabilities. Additionally, we need to ensure that you understand the criteria of the grant. Please note that all applications must be submitted no later than February 28, 2007, to be considered for a grant.  Applications received after February 28th will be excluded from consideration.

Completion of the information sought here does not constitute a contract or any binding commitment, either on the part of your practice or the Center for Community Health Leadership. This form is simply necessary for the Center to determine the total value of the grant being requested in your community.

After all information is received, each individual medical organization applying for a grant will have the opportunity to review the Misys Healthcare Systems Master Agreement with all related terms, including financial and legal requirements related to the grant. At that point, all individual medical organizations will have the final choice to participate or not participate in the program.
